Bloodthirster returns to its original identity in patch 7.3: lots of AD (75) and lots of lifesteal (15%), with no crit and no Health. It is no longer limited to crit champions.
The item does not shine through a single explosive spike. It makes the carry more playable over time: 75 AD, healing on every attack and a shield that fills with overhealing.
It is a solid buy for champions who hold front-to-back fights with their basic attacks, crit or not. It is weaker if your damage mostly comes from spells: lifesteal does not heal off them.
