The item does not shine through a single explosive spike. It makes the carry more playable over time: 75 AD, healing on every attack and a shield that fills with overhealing.
It is a solid buy for champions who hold front-to-back fights with their basic attacks, crit or not. It is weaker if your damage mostly comes from spells: lifesteal does not heal off them.

Lifesteal, a new stat in patch 7.3, only heals on basic attacks, on-hit damage and abilities treated as basic attacks. The item therefore mostly helps champions who attack a lot. Ichorshield: overhealing from lifesteal becomes a shield, up to 165 to 345 based on level.
