Should Rammus always rush Thornmail even when the game is not won only through anti-heal?

Not automatically. Thornmail remains an excellent standard into AD/sustain, but Dead Man's or Force of Nature can come first depending on the real game condition.

Randuin or Dead Man's Plate second on Rammus depending on whether the carry or the pathing decides the game?

Randuin if one crit carry structures enemy threat; Dead Man's if you mainly win through gank tempo and pressure angles.

When should you truly break Rammus's armor-first build?

When AP burst or magic poke already decide fights before your anti-AD specialization has time to matter. At that point, Force of Nature often needs to move up the order.
